I need to build email form ( template) in Outlook 2017 , to allow users to raise change requests. The form has to be dynamic in that it has to have a work flow build in it. So , the requestor raises the request ( description etc) , the PMO added the change request number , it goes to the technical team to impact access, then to to the change board for approval.

- I am open to ideas on how this can be implemented or if there are tools that I can use .
