Fill in a Spreadsheet with Data

I need 4 selected pieces of data from a PDF chart transcribed into a database (over many PDF charts). I have created a web page that has a link to the selected chart and displays a form for the data entry. The webpage will then put that data into the database. All you need to do is open the PDF, find the correct data and transcribe it into the correct place on the web form. Then submit it.

Each chart should take about 30-90 seconds to correctly transcribe (there's actually not much data to copy over), but there are a lot of charts to go through.
If selected, I will give you an "assingmentId" to put in the URL and that will allow you to submit as many chart-data-entries as you would like (and therefore be paid for them).
I can pay $0.15 per chart, so if you want to transcribe 100 charts, then please bid $15, which should take about an hour.
