Need a employee database, must be able to be user friendly for non access persons to enter data, data includes but not limited to: *Employee Background Information* Employee ID, First Name, Last Name, Age, Position, Last place of employment, Contact Number, References (this would be and word doc attachment), Photo of Employee All this data would be. Another aspect would be the *Employee Medical Information* Allergies, Blood Type, Medical conditions, Fit for work verification (this would be and word doc attachment) As well as **Employee Personal Information** DOB, Qualifications, NOK Relationship, NOK Name, NOK Contact, NOK Address, Passport number, Passport expiry date, ID card Number, Drivers Permit Number **Employee Training attended** (Year, Month, Duration date)- show the period, Course Name, Location ,Remarks, Cost of Course *Other related docs* Insert Cv , Insert Blank both will be word or PDF is possible All the double ** would require a passport to view the fields if possible, a search engine to locate the info, a form to enter data, All data entered must have and identification code in order to enter the data and to know whom enter it.
