Build me a simple feedback system for service

Based on the requirement, we think it would better to develop a Webview App for Android that would run a mini mobile site (in PHP/MySql) to show the user interface and save the data in MySQL. The Admin backend/cms part of the app will also be in PHP/MySql and will be accessible from Web Browser to manage all data. The picture shown in the mock screen will be single throughout the system and the questions will appear above it? All questions will have same answer options (star ratings)? Will users need to login before giving start rating? Complete details will enable me to assess scope of work precisely. I will revise / confirm my bid accordingly. Detailed specifications and inputs will also help me make an informed commitment and deliver as required. I ensure a very thorough job and also support if you need changes / enhancements in future.
