“Pay Now” Paypal Button Form functionality

Need the html code for an input form that uses a standard paypal “Pay Now” button functionality with the following characteristics:

Functionality:

Above the paypal button there must be five (5) user input fields labeled “Question 1”, “Question 2”, “Question 3”, “Question 4”, “Question 5”, respectively. Each one of the input fields should be limited to 50 characters in length.

When clicking the “Pay Now” button, the functionality should first check for each and every one of the input fields to have at least one alphanumeric character (A-Z, a-z, 0-9). If this condition is not met, then a paypal payment transaction is not conducted and the UI stays in current condition with no change. If this condition is met, then a paypal payment transaction is conducted, and the content of the input fields is sent, along with the transaction information, to the email of record for the specific paypal merchant account id (given in numeral 3 below).

When a paypal payment transaction is made using the paypal button (after the conditions above are met), the money must be transferred to a predefined paypal merchant account id. The code should be flexible so that this merchant account id can be changed to other merchant account ids and therefore transactions can be made to other third party paypal accounts. To do this, you must indicate where in the code this paypal merchant account id resides, so that it can be easily edited.

Aesthetics:

The form should be simple, visually pleasing and with a professional look. Of neutral colors (black/grey/white). It will be used on a white background.

Other:

The html code should be ready to be pasted into third party websites to provide them with paypal payment transaction functionality.

Notes:

The html code for the Paypal “Pay NOw” button is already created. You can get it for free directly from PayPal. That code should already be embedded in the code that you are providing.

In order to accept completion of this product, I will test it with multiple (at least three) paypal merchant account ids to ensure that all the features asked are working correctly and money is actually transferred into the respective paypal accounts.
