Create a page form in Microsoft Access or Excel macros for Data Entry

I am an independent mobile App developer. I want to create a database of workouts for a new fitness app. Instead of manually entering, copy and pasting each variable in a string of code, I need a form created so that multiple variables can be entered by query's on a form and then once all of the variables are entered the output is a complete string with remaining constant characters and words. I am attaching an example of the output. Each output between the {} is the output of 1 form completed. I was thinking I can copy and paste outputs of each form to create this document. It would be nice if the form can keep compiling to create this exactly how you see it automatically. In other words, this would be three forms with variables entered and compiled. The output would ideally be in a .txt file. For an example a form would look like this: Name of workout? ANGIE (or can be left blank) What Equipment is needed? PULL-UP Bar Additional Equipment? Yes What Equipment is needed? ABMAT Additional Equipment? No How many workouts? 4 Workout 1: Pull-Ups Quantity or weight? 100 Workout 2: Push ups Quantity or weight? 100 Workout 3: Situps Quantity or weight? 100 Workout 4: Squats Quantity or weight? 100 Timer: FOR TIME How much time? 0 (this could be any number) Are you a beginner or experienced?
