Develop a proof of concept app for blockchain company

I need a proof of concept developed to bring an idea to market. This should be a web app for the sake of an MVP. The purpose of this mobile app is to incentivize & reward healthy behaviors of users. This app will generate a health score for an individual using data from popular health trackers and wearables. The health score will start with simple metrics: steps, calories burned, resting heart rate, active minutes, sleep quality, blood pressure, and weight A user’s score will range from 0-1000 and will fluctuate based on activity levels over time. Metrics will be measured and the user will be rewarded with our company's native cryptocurrency when certain metrics reach a target. The algorithm to calculate this score is already developed. A user will access the app and create an account using their Google or Facebook account or with an email & password. After creating an account, the user will be prompted to connect their desired health tracker/wearable. This will be done using Human API ([login to view URL]). After data is connected, the user will be prompted to complete their profile by imputing age, gender, height, and weight. Next, the user will be taken to a page that displays their health score.
