Just by looking at the attached images there are some obviously suspicious stuff in this design. Section 12 of BQ25792 datasheet provides a detailed example on how to arrange the chip and how to do proper routing to all passive components. Also there's via stitching on the reference place which I can't see on your design. There's a strange Y-junction.
