Proposal generator

I am looking for automatic data population of a template spreadsheet given a table of data and a series of input questions which determine the data from the table to be presented. We are a dental benefits firm that sends out dozens of quotes a day to various prospective customers. Our rates are standardized and currently set up in an Excel file. When it is time to do a quote we ask several questions, such as: Where is the potential client? - rates are based on two regions. What plan maximum do you want? - we offer 3. What plan do you want - we offer two What plan design do you want? we offer 4 The answers to these questions allow us to look up the rates on the table and put them in a template proposal spreadsheet. This is a very simple process but time consuming as it is voluntary. We'd like to have an input page with the questions above and a couple others like them. The input page would automatically look up the correct rate and use it to populate the spreadsheet. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform The application would run on Windows XP and just prior.
