vba Excel/Access Data capture form

Building Works Survey/Proposals I have a requirement to caputure data for building works surveys. We currently have 5 paper forms all with aproximately 50 questions on each. However the majority of all questions are common to each form. Each for is for slightly differing types of proposals so some questions do not appear on all forms, etc. I would like a form written in to either Excel or Access. The form should contain all questions but have a selection for the proposal type that will disable questions unrelated to that proposal. Additionally, it is also a requirement to attach digital photos to the proposal and have the optional ability to associate a photo with any of the questions. This needs to be very simple for the end user. Once the form is completed, it will be emailed to a central administrator who will import the data and photos in to an Access DB (likely to be migrated in time to a MS SQL database. The coder will review the 5 forms and create a Form (in ecxcel of access) to catch all required questions for each proposal type (currently defined by the paper form selected for each proposal). The form should disable unrelated questions to the chosen type of proposal. When a form is closed, the form should show a pop up to inform the user about incomplete questions. When the form is closed the file should be saved with a file name of ProposalID_Timestamp.??? If the Images are not embedded in the file, then all associated files should be in the same folder making it easy to ZIP. A macro/vba app should be created to import data and images in to a master database. The coder should bear in mind the app will most likely be ported to MS SQL master database in time. ## Deliverables The coder will need to create an appropriate database/excel template to meet the requirements of this project. The coder will create the master database appropriate to **catch and review** data from all proposals. Including photos,etc. The chosen solution will be supported in MS Office 2003,2007 Operatinfg Systems XP Home/Pro, Vista, Windows 7 The coder may like to consider audit option for changes to the master database. I would welcome ideas to make this very simple/user friendly
