PHP GUI to fill in necessary fields on a form

(ref:[login to view URL]) We need a simple GUI for a project where the user has to fill necessary personal info. The GUI will be called with an array where the info is; will check the necessary fields; if there are some missing entries, invite the user to fill them; store the data. The code will work on a touchscreen, therefore a configurable virtual keyboard that we will supply is going to be used. The details of the module are in the attached document. Some web services are to be used in the GUI, they are already coded, we will provide those to the coder. Use of CSS (Cascade Style Sheet) is a must. All text formatting, buttons, backgrounds and other visual preferances must be defined in CSS. We will provide a basic CSS, in case it is not enough the coder will add his own CSS file with a documentation. The coder will be given an ftp account to test the code also access to the db. The program will run in Turkish language, therefore windows-1254 or 8859-9 sets are to be supported. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform PHP 4.x, MySQL 4.0, Apache 1.3, Internet Explorer 6.0 + versions, Firefox
