1. The custom form for the Mold samples and test results as described in the video can be developed. 2. The form details as described are: a. The form is analytical request form for mold and tape sampling. b. Logged in Users input the following data into the form: c. If the user click on the Mold link the first section will be the customer code, numbe rof samples, multiplying factor d. Upon submitting the second section will be displayed. - it contains Sample type ( non rush tape lift sample, rush tape sample, non rush aircell analysis, rush aircell analysis etc.,) Customer code and details from the loggedin information is prefilled in the form fields. e. User selects his option and submits. f. Assign COC (Chain of custody) and the Invoice details. g. Mold Analyze: Displays the mold data that has been inputted. To show the analysis and the presence of the various organisms in the samples.( as per the excel provided). h. Mold check : Displays the samples that has been already saved and completed. i. Mold Reports: To list the molds that have completed their process ( similar to archived ). 3. The solution can be developed in WordPress with custom plugin and form.
