We need an interactive form we can use to conduct on site safety assessments for swimming pools/spas and fencing. This document will be used internally and a copy will also be given to our customers so it needs to have a professional look and feel and be consistent with our branding in terms of colours, fonts etc. * You can use the form building app of your choice - however one example would be FastField. * For our safety inspection checklist, we require the following features; - Multi-platform support (eg iPhone, Android, Web-based) - Easy to use, printable (Portrait format) - Allow for note and photo taking in the field (Notes and pictures must also print) - Share form with customers (eg PDF version or web-based link) - We would also like each report to have a unique identifying serial number and all pages must be numbered and dated. We have a rough draft of the form we which you to use to create our interactive form. - Attached is a Powerpoint document taking you through the flow of the form and some of the changes we wish to make to the form as it stands. - Please note, the draft document is currently in landscape format, we would like the finished product to be in portrait orientation. - There is also a word document attached, please use that and check the highlighted sections to update our draft as well. - We have also included our business logo which must be included on the final report. Please use this logo to guide colours and fonts. * Ultimately, we would like to have full control of the final product and be able to make changes to the form (wording, easily add/remove sections etc) ourselves.

Hi, I had a quick overview of attached doc and PPT file. From the description it seems it will need to be built custom from scratch since you want to have full control of your product so using a third party tool like you mentioned in description doesn't fit. I am thinking of WordPress at this moment. There are some other options like making it in Laravel is also feasible. I'm a web developer and designer as well.
