Build PHP/Mysql form

We need a developer to create a simple settlement booking form which will then need to sit inside a widget iframe. We have tried doing this in google forms, but we need a better hosting platform for sensitive data. The application should be in two parts for Internal users and external users Internal User - Administration Create a booking by filling in the below information (a) time frame for day of settlement – eg 10.00am to 3.00pm, include lunch break if required (b) duration of each settlement time slot, eg 15 mins (c) number of settlements in each settlement time slot – eg 5 (based on staff available to conduct each settlement, differs per settlement date) (d) Title – [development name] (e) Property – [address of development] (f) Settlement venue – [text] (g) Settlement date (h) contact – name, phone, email Internal user - View / Add/ Edit/ Status booking details Report on booked / not booked External user booking request Only relevant settlement date available to external user Indicate to user the available time slots Indicate to user the remaining number of slots for each time slot External user booking request User to select time slot, then complete the following information on the settlement booking form: (a) Lot No (mandatory) (b) Purchaser name (mandatory) (c) Contact details – Name, phone, email (mandatory) (d) Additional information User to click Save or Cancel User unable to amend or remove existing settlement bookings Email Accept / Reject Process With ability to resend emails When multiple lots - all details will appear in one email Rejection sends email to Property lawyer to follow up Administer / Internal Users login Internal users need to login
