Build a simple website for doing survey/test

I work for a small company and our group has about 50 people. Currently, my group wants to build an internal website to do some surveys. We want to, in particular, put some short questions on a website (we will use a server we have) and then send a link to a people, then ask him to answers questions and leave his name after he finished. We decided to build a website so that we can put some questions on the website. So I'm hiring someone to help: 1. Write a prototype on a personal computer, so I can run the survey/test locally. 2. The prototype should be written in Python. 3. I am a reasonably good programmer using python, C++ but not familiar with how to build a website. I will need to communicate with whoever wrote the code (for a couple of hours maybe) and try to understand the prototype for the maintenance purpose only. 4. The prototype doesn't need to be too fancy but aims to be a one-day/two day project. Welcome to contact me so we may have a short skype chat.
