Want i learned from your description is : You want some form type which needs to be built in html The form needs to upload the file (in csv format) When it is submitted the entry is made with the unique key and the file is uploaded in the next column . Meaning by the sql table is needed to be made for this containing 3 columns. Column 1 is the unique key Column 2 is the name of the uploaded file Column 3 is the description/definition of it.
