Upload data from Adobe Forms to SQL table via website for manipulation of data

I have an adobe acrobat pdf form (attached) that requires once completing by the end user uploading to an SQL database via our website.
Initially, there will 2 ways to enter details to the SQL TABLE which need coding. These are as follows:-
(1) By dragging and dropping the form to a field on our site (yet to be built) - this however requires 2 ways of “uploading” in that we need the form to be “dumped” by either an agent who logs into their account – and it records the agent – or “DIRECT” by the client who completed the form. THERE IS AN AGENT REF so ……….
(A) If agent Ref = Blank, DIRECT BUSINESS
(B) If agent Ref = 1234, then AGENT = BLOGGS AND CO (TAKEN FROM ANOTHER TABLE ON LOOKUP
(2) By entering in an HTML form basic details of the client and mapping to the SQL TABLE details – these details will be a limited version of what is detailed in the full Pdf Form but also the ability to upload documents (word/pdf) as some forms will be other applications that are simply for our information*.
* This section will need completion by either an agent or DIRECT – so if agent, this would be best undertaken following a log in by the agent/verifying from AGENT TABLE
Both methods also need the ability to upload additional documentation and then to be linked to the Table for our future use.
This is the first stage of what is intended to become an online portal to manage and process new enquiries to our business.
