WordPress Site - Plugin issue with Responsive Behavior

We have a worpress website that uses a custom Plugin [login to view URL] and we also use, by the same author a Plugin add-on called Extra Forms fee-able options [login to view URL] Our problems: When I activate the fee-addon module The pay now and pay later button no longer work Also when that module is active I loose the mobile-friendly aspect, responsive for mobile the circles or the "beads" just keep spinning and don't stop. Also that forms extra fee-based add-on appears to only allow one field on an invoice and we need the ability to offer more than one fee-based add on per invoice. our website is [login to view URL] and this module can be seen on our events and schedule page in April. We need someone that can fix this module or enhance it. Please do not bod this project without reviewing our needs specifically. We will not pay unless you can complete this project.
