Build a form for our website that looks like TypeForm, Using Authorize.net, and template

Hello, We are looking to have a signup form that looks like a [login to view URL] form, however, they are way too limited. The Two advanced features we need to be added are: reCaptcha, Credit Card Processing, and a scheduler. I want the scheduler to look like the one found on [login to view URL] Our form will connect to our API to create a user, charge a card if needed, and schedule an appointment. All you will need for this project is CSS, HTML, JavaScript, AJAX. All PHP work will be done on our end. For the schedule, you will pull a date from our API for example 10/18/2017 and it will return how many appointments there is that day. And same for the exact times, it will return looking like this ['2016-03-04': ['1': [['09:00:00','09:30:00'], ['11:15:00','14:45:00']] , ...], ...]} . The project is complete when: The form works with: - Check for valid email, phone, etc - Fallback for invalid or already used email, credit card, - Submits data to our endpoint - Runs on all major browsers and mobile devices - Code is documented and clean You may request an example of what I am looking for, simply ask in the bid and I will give you a login to a TypeForm in an hour or less.
