I need a two different pages designed.

These pages are for use by a quiz master.

Page 1: This page should include the questions and also the answers for 10 trivia questions. There should also be space for the inclusion of a url next to the answers.

Here some examples of some questions and answers.

Question 1. The longest route for one train is where?
Answer: between Moscow and Vladivostok on Trans-Siberian Express railway line that is 9,297 kilometers long!

Question 2: Name the first two countries in the world to allow women to vote.

Answer: New Zealand and then Australia

Page 2: This should be the page that the participants fill in when answering the quiz. There should be space for a person to enter their name, area to write the answers and also an area next to each questions for a tick or number of reflect whether it is a correct or incorrect answer and then space at the bottom of the page for the total number of correct answers on the page.

Both pages should include reference to triviameister.com in a bold way so that it stands out. It can be in stylised text or as a logo (that you create).

It should fit on both A4 size as well as US letter size paper.

Also, in the same style please create the front and back of a playing card size card which has one question and one answer on each card. The backside of the card can be decorate or include the triviameister.com logo.
