convert an online form to pdf file

I have a form currently on jotformpro.com. I want it to be downloadable, writable and printable pdf to look exactly as it currently does on jotform. Some fields are drop down, some have comments. I want to put it on my ecommerce site as a downloadable document instead of accessing through third party. It has to look the same as it does now -same format, same comments, same colours.
