Set up my online form with Spry Validation - then get it submitting properly

I have an online form that needs to set up with spry validation. It has 24 questions, with a mix of text fields, check boxes and drop downs. To speed things up I have already set up this html form on a page, and created all of the fields in an html table. The test page is currently online.
The form needs to spry validate each field as the user progresses forward - and display error messages next to each field prompting the user to enter correct information where required. I have created the online form (it is currently in a .php file) using an html table, and set up all of the fields marking them with red developer notes to show which ones are required, and what the spry messages need to display.
This job requires you to be able to:
Change my code where there are errors in my supplied set up
Be able to set an "email address match" confirmation function to verify the user has entered the right email address part way thru the form set up
Provide a simple date picker for a birth date entry
set the clear button to clear the form
set the submit button to submit on blur
test the form and verify it works both in validation and that it sends information correctly to an email address (using formmail currently on my site)
