484103 Correct Simple Form & sign up for newsletter on Mailchimp

I have created a form that I need someone to help me correct or start from scratch. I will attach the code I already have that way you can get an idea of what I am talking about.

All fields need to be required except "Company Name, Job Title, Address line 2, & Country"

Email address field needs to be validated to make sure that it is a valid email address in php & java script, and a pop up window or some sort of text needs to be displayed saying which field is not correct.

There should also be some sort of identifier showing which field is not correct, maybe highlighting the field or changing it to bold so that the person entering in the information knows exactly which one it is.

I also need the check box to automatically add these people to our Mail Chimp newsletter form. I am attaching the code for the mail chimp form ([url removed, login to view]). You will have to use some of that code to be able to add them to our news letter from the form you have created.

Lastly I have added some new field to the form but have not added them to the database field or in the code because I was not sure how to get them to work with the mailchimp form so if you could also look over that as well.
