Wasy - Quick - Web page to display interactive google map

i have a need to create one page that will show an interactive google map. I have five locations i want to show on this map and i wantthe same functionality is the following page:

I need this done ASAP - i expect to approve this project within an hours of posting it and need it within 1 days time.

This is on a RAILS machine -looking for php or standard html page
