Create form in TYPO3

The website is a TYPE3 -Test website. You will get full access to it. We have 10 questions which will be dependend from each other. Sample:
Single / Married >
Single > Select country
Married > What is your wifes Name?
If Wifes Name is "more than 10 chars" > Select wifes country
...
If all is valid
submit form to database.
On the TYPO3 page is also needed:
- show form data of database
- send email
- create PDF of formular
At the end of the form-project it will be setup on real website, not test-page.
I need someone to explain the steps if needed because he understands it. No huge company and sales person, just a technical experienced guy please to assist me.
