Move a PHP form into a bootstrap web page

I have a application form on my website which is written in PHP, it works fine but the website it's on is outdated so I'd like it moved into a new Boostrap site.
The form works 100% fine and is not complex in the least, it's a quick job for someone with the right skills, however instead of having 4 browse buttons to attach files, I'd like an area you can drag files into OR click into to browse your computer and attach files.
