Need a simple faucet script for altcoin

As mentioned in the title, i need a simple faucet scrip for an altcoin. The script should only have a field to enter a wallet address and a submit button with captcha. The scrip needs to prevent abuse by restricting the number of payouts per day by detecting the IP address. It must be created in php. Design is not required. Simple as that.
