Slightly Modify Algorithm to produce output

I have an algorithm which I have uploaded.
I am looking for it to create five columns and place the results in a Pandas dataframe. That is all. Heavy lifting is done. I have included an image showing what the output should display.
I have attached a csv as an example of what the inputs will be like that get fed into the algorithm.

I have gone through the algorithm. But I am little unclear about the requirement. Is it simply storing the data frame in a CSV format or something else? I want to have a chat with you about the exact requirement.
I have experience with python 2 and python 3. I also have previously worked on pandas dataframe on python. I am confident that I can deliver my project in time.
