Javascript Page to Get information

I’m looking for someone who’s familiar with JavaScript and HTML to help me with a simple page that I want to create. Basically I want this page to be running on a browser locally using MS Internet Explorer or Google Chrome without needing to get it uploaded to a webserver. I envision the final product to be either a 2-pages or 3-pages HTML file. The essence of the pages is to collect unique information filled up by users. Basically I have multiple forms (maybe up to 15), each having a same and different set of fields to be filled up. We are using this set of HTML / Javascript pages to collect data. The eventual HTML / JavaScript page should also be able to run on Ipads / Android tablets. Page 1 – Introduction and Form Selection. At the first page, I will have a short paragraph of words (around 50 words), after which users can select a list of 15 categories of forms to choose from. Users would select from a list of fifteen (15) categories using “Check-boxes”. Each category will have different set of fields that may be unique to that particular category or may be duplicates with other categories. There will be up to 15 categories with each category having up to a possible of 30 different field types. The challenge here would be to write the script in a way that it accommodates this. The script should also be written in a way whereby I am able to modify the 15 different categories of forms and the associated fields (which is up to 30 different fields). I’m ok with this being hardcoded into the HTML script, just provided some “comments” for me to know where to modify it. Taking an example scenario whereby a user selects three (3) forms whereby it has the following combination of form category and field types below, this will serve as an explanation on how Page 2 works. FormCat_1 (a form with 6 fields) · FieldName_01 · FieldName_02 · FieldName_03 · FieldName_04 · FieldName_05 · FieldName_06 FormCat_2 (a form with 3 fields) · FieldName_01 · FieldName_02 · FieldName_07 FormCat_3 (a form with 4 fields) · FieldName_01 · FieldName_02 · FieldName_03 · FieldName_06 Page 2 – Data Input from Users Using the example of the three (3) forms selected from Page 1, the 2nd page should display the following special form for the user to input the data: SpecialForm · FieldName_01 · FieldName_02 · FieldName_03 · FieldName_04 · FieldName_05 · FieldName_06 · FieldName_07 User would then enter data into these fields and click “Submit”. Upon which the Javascript would save a copy into a text file with a filename that corresponds to “Fieldname_01” and a timestamp. Fieldname_01 would always most likely to be the “Name” of the person. If there are other methods proposed to store this data, I’m open to suggestions but bear in mind that the laptop would not have Internet connection (so emailing, etc would not be feasible). For the purpose of development, the developer can use any dummy data for both Category of forms and fieldnames. I will modify this at a later stage to test how easy it is for me to modify it in the code/script/HTML file. Page 3 – Confirmation / Thank you page A very simple HTML page just to say “Thank you” or to confirm the data submitted.
