Create a Custom Contact form in Shopify

My client requires various custom contact forms to be created for their Shopify site. The main functionality this form will need is: The option to attach a file. The option to reply to the customer once the email is received (eg pick up the customers email address from the form and use that as the reply to address when my client receives the email). A custom email subject that takes into consideration which form the customer has used, and the customers name. (eg "Contact Form Name" Contact from "Customer Name") I will go into more detail about the various fields the form needs once I have found someone who believes they can do this.
