Internal Web Based Database For Membership

Overview CWDO is a small non-profit organization that actively promotes the rights, freedoms and responsibilities of persons with disabilities through community development, social action, and member support and referral. Our primary activity is public education and awareness about the social and physical barriers that prevent the full inclusion of persons with disabilities in Ontario. We have 2,000 members across Ontario and need a database to organize membership information. Work includes creating a form members can complete on our website at [login to view URL] that feeds into our Member database. Work includes creating queries such as: - list of members with a disability in ...x... city or electoral district with an interest in ...x... - List of members signed up for ...x...newsletter Note: we are not interested in the visual look of the pages in terms of css. Styling and decorating any work on the CSS/look will be covered at a later date. At present we are looking to put in the base functionality and structure (html/php) for the site. The admin portion of the app should include Add member, list members , edit members and advanced search screen. The app should have the ability to import and export all or a portion of the database as a CSV file. Technology A separate form or submission system should be created using the existing drupal instance. The admin portion of app will be written in PHP and use mysql to store the database. Should the creator wish to use PHP or alternately drupal, that is acceptable as well. Regardless of the system, the App will be password protected. By a username and password which will be stored in encrypted form. All modules to be added to the site should be identified to the site admin prior to installation. Basic Database Elements : See Word Document Add member screen Should allow the importing of records by CSV or the manual adding of a record List members screen Should have a simple search prior to tabular list, all entries in tabular format with email, show record, edit record and delete record links, export to csv option for all in list or all results Edit member screen Should have same fields as Add member Advanced search screen Must be able to do queries with the database with connected criteria – such as everyone in a certain location is interested in parenting, or everyone who was interested in parenting, who also has event planning skills. Search results should use the list screen format for results Note: Milestone for work will not be consider achieved until all work has been done to the satisfaction of the CWDO advisor(s)
