Survey App

We are in need of programming an app (IOS/Android) with the following characteristics and want to know if you are capable and also know what is the associated cost. The idea is an application where you can conduct surveys , surveys should be able to perform without an Internet connection , store data on your phone or device and then be send to a server. We need to develop a CMS where the administrator (a person who does not have programming knowledge ) can modify and develop new surveys tahat are going to be downloded on te app . The idea is to develop question formats within the administrator with the following questions: 1). Multiple choice with one answer. 2) . Multiple choice multiple answers. 3) . YES / NO. 4). Pairing ( Column 1 . Column 2 ) . 5) . Asked by date . 6. Asked hour. 7. Question with image . 8. Case study, where they can put long text in the question and attach images and videos. 9. Multiple choice option "Other" and the space for the answer. 10. True / false with " Why? " . 11. Asked box for row and column ( that's like a match too). Internet is only needed when uploading de answers or downloading de new surveys. The operator that is going to aply the survey, have to log-in, with a username and a password so that way we control de surveys. The survey data must be sent to an email in excel format organized according to answers on the survey. Thank you very much
