2 Landing page form designs 4-5 hour project

We need you to: 1) 1. Redesign and setup the free consultation sign up form that you see at the left: [login to view URL] so that affiliates can place it on their websites (and have it track their affiliate ID) using our affiliate program. We want you to add the CuraDebt logo at the top of the form. This addition must be done with excellent design (consistent spacing and feeling) * Note: form needs to have error checking * Add a hover popup explaining what happens after a person submits the form explaining that the person is transfered to CuraDebt to get a free evaluation * Add something below the form saying "Sponsored form" 2) Create a 2nd sign up form looking like: [login to view URL] (the form in the middle of page but replace the column on the right with CuraDebt logo and add the TASC and USOBA credentials from (at the boom of the this form) [login to view URL] * Should have error checking, the privacy and what happens next as hover popups and the fields from the form on the home page of [login to view URL] * Also be set up and tested for affiliates to use by placing form on their sites to generate leads Attached is the CuraDebt logo to be added to both forms. Please give examples of 2-3 forms you've done the design of (similar to debt lead forms would be great, but not required). *****Preference will be given to designers with beautiful forms and who demonstrate a great attention to detail. Need revisions until satisfied. We also need this done asap so please confirm you can start asap and ideally be done in 3-4 hours. Thanks. ## Deliverables 1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement). 2) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 3) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Employer's environment--Deliverables must be installed by the Worker in ready-to-run condition in the Employer's environment. b) For all others including desktop software or software the employer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this project. ## Platform HTML, web, affiliate program
