I am looking for a developer to create a web-based waiver system that will let my customer's check-in into my business. The person entering the business will identify themselves by phone number, if the phone number is in the system already and the date isn't too far in the past, they will be asked to proceed. If the person isn't into the system, they will have to complete a few fields and provide an electronic signature (mouse or touchpad) On the back-end of the system, a person will be able to verify the status of a check-in by entering the phone number into a field and return the status of the check-in. If the status is good, they will accept the check-in. All the information during the check-in will need to be stored into a permanent database
